amavisd running and filtering enabled are two different things.
The comments in 15-content_filter_mode explain it.

$sa_tag_level_deflt = undef; # always add status

I recommend not discarding spam mail. Tagging spam, delivering to spambox,
or rejecting (only at SMTP level, so you don't backscatter) is much more
safe, otherwise users could blame you for discarding their mail without
letting anybody know.
