gen_mail_id FAILED

I'm periodically seeing this error in my postfix logs.  It's happening
infrequently - on one server, 30 times out of some 54800 emails
processed today - but just often enough that when it does, I'm getting
complaints about mail delays of 5-10 minutes.

Is there some way to log a bit more info about what's causing this issue?

Mysql with innodb tables.
